I just delivered my fourth baby. I have three older girls and this is my first son. I started off using Pampers Swaddlers with him and the same thing happened. I switched to Pampers Baby Dry and he has been fine. He had a red rash in the shape of a diaper with little white pustules/blisters. The new diapers are made different and he has no reaction at all. I also swear by the cornstarch powder for wetness control. Very light dusting. I have used that since my 12 year old was a baby.

To help clear up the chaffing, leave the diaper off for a little while before you put a new one on. Just make sure you have a washable or water resistant mat under baby. The air will actually help heal baby faster than any cream you can buy. My second child was actually allergic to most disposable diapers, we could only use huggies. Ask your Dr. for his or her advice at the next check-up. If the rash persists, it could be something else, such as a yeast infection, which would require Rx cream.

keep an eye on them, if they turn like an orange color than it can be impetigo. when my son was only a few weeks old he got a similar rash in the same area and it turned into impetigo. we just took him to the doctors and they had to open each blister and empty it and then he was put on antibiotics. it can be dangerous if not treated.

i would recommend a check at the dr,,,chafing doesnt cause blisters. could be an allergic reaction. i know to prevent diaper rash,,,i was a powder mom,,,,i know everyone these days say that is a big NONO,,but ill tell you what,,i had boy/girl twins and i NEVER had diaper rash with either one. i used the cornstarch based powder, and would put it in my hand and then on the baby to avoid the "big puff".
honestly a call to the dr would be in order,,blisters are not a normal diaper condition.

My favorite thing to use was A&D because it got a really good rating on the safe cosmetic database and it worked.

You should call the pediatrician. But until then, somehow air the area out. If you can leave your baby without a diaper, even if only for a few minutes, that might help. Also, since the day my daughter was born I've been putting vaseline on her bottom at EVERY diaper change. It's suppose to help prevent diaper rash. OH-use the kind that doesn't have a scent. Generic is fine as well. The hospital told me this.

Maybe try a&d?
I use cornstarch powder as well, and I haven't had a diaper rash or chaffing since. It really does work and I don't know why people are so against it!
As for the spots, my friends son had that happen recently. Her doc said she needed to cange him more often, and keep him dry as possible. After you wipe him down, make sure his bottom is dry before you diaper him. Call your dr, office or maybe the hospital.
Hope he gets better soon!
